Finally married! and some updates

Assalamualaikum to all my silent readers (you know who you are),

Alhamdulillah, I'm finally married on the 29th December 2018 to my love, Syeqqal. I'm actually very grateful that Allah unite us together in this sacred marriage. May Allah grant us mawaddah, sakinah, rahmah and barakah till jannah, and may we overcome any struggles ahead us to become stronger. Although we only have known each other for 6 months, but always remember if Allah wants something to happen, it will surely happen. All in His great knowledge because He knows what is best for us. So, have strong faith in Allah!





Seeing all the wedding photos, rindu plak. tbe2 terasa diri cantek plk. hehe, thanks to the make up artist eizwaniehnm (ig) and to my baju pengantin and the best pelamin by my kakak ipar, here's some of the photos. Also, Alhamdulillah selamat da majlis bertandang kat perak. rombongan cik kiah jgk smpi satu bas p perak. short trip for the whole family but so glad that they really enjoyed it. Alhamdulillah.




On the other side, today is my 14th oncall in O&G as a floating mo since I started oncall. there's still a lot more to learn and practise csec etc. a lil bit exhausted actually lg2 ble berjauhan dgn suami. kiki. berkorban ckt la Latifah. org lain pjj one month or more, kte ni seminggu sekali jumpa, harus byk bersyukur ye.

There's so much to learn not just for my o&g posting but also in married life. huuu.. I guess, I really need to lower down my ego, to have high respect and listen to my husband. hee.. will work on that slowly. so much of adaptation to do.

On a side note, it's been nearly 2 months floating in O&G as a MO. honestly, the excitement to explore a lot of things have slowly faded away time by time. I'm not sure whether it's just inferiority complex or jealousy seeing your friend have achieved so much more, first call suda. takpe la, slowly latifah. improve yourself bit by bit. need to speak up clearly, confidently and calmly.
Bak kata cik suami, beranikan diri. Tiada kuasa lagi besar dari Allah. ingat tu latifah! La hawla wala quwwata illa billah.  May Allah ease the journey. xpayah nk fikir sgt ape org duk kata, just do your job. tunjukkan akhlak yg baik instead. hmm. easier said than done, but I'll try.

lets just pray hard that I will be posted to somewhere near to my husband and family. Aminn, can't wait to be out from the toxic environment, sorry to say. hang on for at least 2 months latifah.

Miracle

Assalamualaikum,

Hi everyone. Hope everyone is in good health. it has been almost a year of idle, no update what so ever. sorry for that, was too occupied. ni pun patutnye study. curi 10 min jp update blog.

Well, a lot of things happened in my life so far. so much changes that I don't know where to start. hee. mostly it's about my love life. I still remembered it was raining quite heavily talking to my pak itam saying that "sedihnye etc" because baru lps kononye break dgn one guy yg baru kenal. so sedih that I thought the rain understands my feeling. then, the same night, someone texted me through some social media saying "Assalamualaikum, salam perkenalan. selamat hari raya". padahal dlm hati almost nk give up.

He was a complete stranger to me. a year older, wiser ckt,same wavelength of thinking, rse bole borak mcm2 dgn this guy without feeling awkward, loves music and is a guitarist (dedicate songs to me at the very first time he called me), so brave and honest, determined, understanding, knows a lot about car etc. His determination to get to know my family and made the right move to see my parents yg buat den jatuh cinta. hee. n everything else. hahah. (xleh cte sume segan plk)

and story cut short, we meet up once, then I brought him to see my parents n family. 3 months after that we got engaged. Alhamdulillah.. and now 70 days left for us to get married. dubdab..hehe


some of my friends said, lajunye, ekspressnye, yakin ke? yes, In sha Allah. so far, Alhamdulillah sume urusan dipermudahkn Allah. n we hope to bercinta lps kawen, walaupun da tercinta awl plk. hee.. (tutup muka)

almost 2 weekly akn jmpe since first me met, so when it is more than the usual gap. the feeling of rindu menebal plk. hrp2 dpt mo placement dkt dgn bkl suami n family den. Aaminn. cannot imagine if terpaksa pjj. huhu..


tlg doakan yg baik2 utk kami ye kwn2. smoga dipermudahkan segala urusan kte sume, dipertemukn jodoh msg2 dan smoga sentiasa dlm lindungan rahmat dan kasih syg Allah. one important lesson learnt, trust in Allah (tawakkal) and never lose hope. tp at the same time kne ade effort jgk. 

viva in less than 2 weeks. and still so many topics to study,

It has been a year of HO

Assalamualaikum n hi everyone,

(lap2 habuk tebal kt blog bersawang ni.)Alhamdulillah, genap setahun jd houseman n Alhamdulillah dpt completekn 3 killer posting Medical, O&G and paeds this year. Only Allah knows all the struggle, hardship, sweat and tears, kilos of weight loss during the past year.


But also, the joy of seeing your patients recovering with Allah will, the happiness of getting the blood from the first poke or after few attempts , receiving gratitude from the relatives for taking care of their loved one and others. Not to mention, all the scoldings which actually make up the learning process to become a competent doctor, sleepless nights where you have to be on foot and attending unstable patients till morning and then rush to the clinics to see patients when all you think is sleep. (pernah jgk hampir tersengguk dpn patients) sorry makcik! and also the endless drama in hosp which I'll not describe further. hahaha

So, things have changed a lot for the past year. I have moved into  a new house since my exhousemate just get married and have moved to another house (actually sebelah rumah). Having new housemates n new environment, just settled and I'll be entering a new posting this Thursday, surgical! woot2.

One of the best doa that my friend shared to me which I recited everyday. Also amalkn bace alfatihah,ayat kursi, surah dhuha, doa p keje ni n selawat byk2 sepnjang jln nk msk hosp tu.




So, I just would like to share some of the tips and tricks and maybe what are the few life lessons which I gained from each posting. Let's begin shall we:

MEDICAL:

huge workload, have to mentally and physically prepared to go through this posting, Because most of the days, the ward will be busy, never ending admissions, blood taking for the whole ward (30 patients), postround order where you have to run to the radiology department requesting for urgent CT brain to rule out for haemorrhgic stroke (example), assisting your MO to perform LP, doing CPR at the early morning and breaking bad news to the relatives. this are of course just to name few. because ho jobscope is kinda wide, doing discharges properly, review patients, clerking new patients and giving a proper diagnosis and managements!

So, this is where the knowledge, experience and skills came along. I learn a very important lesson which my specialist once said "you are a doctor too!" after trying to rectify my mistakes that I depend on the management of my MO too much where in fact you should be also actively manage that unstable patient. find the cause! not just giving conservative management. for example, if patient complain of shortness of breath, don't just give oxygen! find the cause of SOB and treat the underlying cause!

Priority is very important! Teamwork is second. Be a good teamplayer, a leader and a humble and eager leaner. READ3X all the common topics especially after encountering that case. for eg pneumonia, what the managements, TB, COPD, asthma, Acute pulmonary oedema, MI and others.


O&G:

Workload is actually bearable because there's only 4 important questions you need to ask, contraction pain, show, leaking, good fetal movements? and of the other relevant info. investigations, examinations and management also quite similar. for eg, early labour, after ve if less than 4 cm, send to ward for FMC/LPC. shortforms mmg byk. memula asyk dok tnye je sbb xtau.lama2 nt ok iA.

Tips no1: learn to work fast and efficient from clerking, examinations, inv etc. because every seconds counts. kalau lmbt inform if ctg xreactive abes la.. kalau lmbt inform pt high BP, n xwt pape abes jgk. basically before nk inform pape, make sure do the necessary things first, examine, know the case etc.

tips no 2, read up the latest guideline RCOG, NICE, perpetua etc. cover all topics in logbook especially the common ones, pre-eclampsia, miscarriage etc. and kne ade good teamwork jgk.

PAEDS:

Babies are cute and very special in the type of fluids, how they present, managements etc. so, need to pay attention to the history. from presenting complaints smpi la diet history sume kne specific. kalau x, abes kne bambu. kiki. sbb sume benda ni akn help with making the correct diagnosis, adakah dia ade failure to thrive secondary to inadequate carbohydrate intake or too much milk taken a day etc.

common conditions cm asthma kne cover btol2,thallassemia, nephrotic syndrome, bronchiolitis, pneumonia to name a few &other cases dlm logbook tu. haha. learn to examine child confidently and properly. build rapport with them. again CME and teaching xbole miss. very important utk viva etc.

last but not least, drama in paeds is the norm. bare with them n just follow je ape depa nk(harap xd yg bce blog den) haha. n in shaa Allah, you'll survived.


ok rsenye tu je setakat ni. gotta start some serious reading from now on, as my specialist already ask how much is much you wanna learn in terms is skills, knowledge etc? hahaha..

preHO Crash Course

Assalamualaikum and hi,

Alhamdulillah dpt pegi preHO crash course last weekend. totally recommended and worth the money. The course helps us to know the bigger picture and setting a positive mindset before entering housemanship especially for overseas students. First and foremost, have a positive mindset even before starting HO. Set the right intention.

So, let me share a few important tips to excel in housemanship so that it will stick in my brain better and for others to benefit from it.

1. Try new things
2. Ask to the right person
3. Never defend yourself. Boss is always right!
4, How to cope with anger: Breathe, Let go and Smile -BLS
5. Aim to become an independent MO at the end of housemanship. so, learn as much as possible to assess and manage patients competently.

How to choose hospital?
-choose the nearest one to your family.
-secondary hosp have less HO, so more chance to perform procedures
-hosp uni usually HO are very less, so there's a lot of work burden and less chance on hands on skills to do procedures because sometime only MOs are allowed to do certain procedures like LP.

HO system in Msia:
These are the flow, we will either being posted to do Medical first, surgery, O&G, paeds, ortho.or Paeds, ortho, O&G, medical, surgery.
1. Medical/ Paeds
2. Surgery/Ortho
3. O&G
*make sure no extension in the first three departments and hopefully none!

Working hours:
Oncalls:

  • 1 week, 70-75 working hours
  • less than 24 hours stretch call
  • proper day off
  • usually oncall when there's not enough HO
  • rate ikut flexi hour
Shift:
  • Mon: AM 
  • Tues: AM
  • Wed: AM
  • Thurs: ON(on night) 4pm-10am tomorrow 
  • Fri: NO(night off)
  • Sat: Day off, but still kne attend CME at hosp
  • Sun: AM
Tagging:
We are expected to be a functioning doctor from day1. So learn as much as possible during the tagging period; hosp system, write patient notes, order bloods, which bottle for which test, clerk patients and know the patients in the ward eventhough they are not assigned to our cubicles. We need to be proactive and train ourselves. Ask around whenever in doubt. Always remember to introduce ourselves. Learn to organize and prioritise the work. 
Time: 5.30am - 10pm
after 2 weeks, there'll be offtag assessment- to prepare ourselves to do oncall/shift on our own

Ward routine:
-blood taking
-Rounds: do review, latest inv, progress/deteriorating, doc, PA
-Procedure
-Admission
-Discharge

Common mistakes:
1. Don't know the cases
2. Round not finished
3. Order not done
4. Important result/progress not informed

Jodoh rahsia Allah

Assalamualaikum,

Hello readers.. hope everyone is in good health, happy and content with life. So, I've been thinking a lot lately about 'jodoh' which I thought it would be nice to express it in words, since I'm already 25 years ago (still 24 though sbb blm birthday).=p and it seems like everyone else is either getting engaged, married or having baby. and I'm still here. and my parents are starting to get anxious more than their daughter, wanting to help in any way they can. but, it's ok, let's hope and pray for the best.

Let's begin with this superb advice which I encountered in Twitter;

So much truth and wisdom in this simple advice. Be a woman. Commit to your work, commit to your parents. We need to learn to build ourselves, our career and most importantly our deen. Hoping along the way, our jodoh will come knocking at the door. ehemm.. hahaha.. so, sentiasalah usaha and doa yg baik2. 

On a side note, don't buried ourselves in the past. All the experience are our best teachers to remind us that if we depend on humans, humans can disappoint us, break us apart, make us sad, frustrated and depress. But, if we depend solely on Allah, we will feel contented because even when our plans didn't worked out as what we want it to be, remember that it's Allah plan which is the best plan! for which we might not know the hidden silver lining. Have faith, always berbaik sangka pada Allah. 


"Stop being so hard on yourself. Everything will make sense to you one day. All the pain, hurt, and frustration will become worth it. Remember, everything happens for a particular reason. You are in the exact place you are meant to be right now. So breathe, be patient, and trust the course of your life. Let go of all the expectations you have created in your head. You are so strong my dear. You have been through a lot and come out on the winning end. The world has tried to break you, but you never let it. Thank your past, for it has made you into a better person today.

Not everyone you have meet is meant to stay around forever. People will leave you. On the contrary, you will also leave others. People will disappoint you, and all you can do is learn to appreciate them for what they have taught you. Thank the people who have stayed. Fill the spaces of the ones who have gone.

More importantly, gracefully let go of the things and people that are not meant for you. Accept things the way they are."

Some updates. =)

Assalamualaikum,


Selawat dan salam pd junjungan Nabi Muhammad SAW. Alhamdulillah ats segala nikmat yg Allah kurniakan pd kte sume. baru perasan setiap tahun semakin bekurang blogpost, dari memula 76 sampi 3 post je utk 2016! haha..So, here's some update for ye. campur satu, dpt la 4. kiki



It has been almost 4 months since I said goodbye to Ireland. missing everyone, every little moments spend together kt Ireland, magnificent scenery and everything that could not be expressed by words. kemain rindu.. Alhamdulillah, I have graduated on the 27th May 2016. Words can't described how grateful I am after checking the results. Because every medical student would know how hard, intense, mentally, emotionally and physically draining course it is, to go through every little test, to preservere until the end, to still hope for the best when you have doubt on yourself and every other struggle that only Allah knows.

For my dearest friends who are still struggling to finish their final exams, I wish you the very best, May Allah grant you perseverance, patience, strength and sabar to go through this final hurdle in this part of the journey. Cikit je lg tau. Stay strong and keep it going. La hawla wala quwwata illa billah. Same doa goes to all the final meds..besar da korg sume, nk jd doc da.. Keep calm and do your very best. Help each other and doa byk2 dgn penuh pengharapan pd Allah. Allah Maha Mendengar.

Back to the updates, so before my parents and I went home, we went to a few places in Ireland, London and Scotland. It was just a short trip but it's full with important lessons in life. Because it's a different experience to travel with your parents compared to your friends. Not saying I don't enjoy travelling with them, but it has different story and struggles in it's own, which I could sum up into my elder brother advice: Sabar seperti mana mereka sabar jg kte sejak dalam kandungan hingga besar ni. Syahdu plak.. But, I am glad they enjoyed their trip.

 

Since we just stayed for one night at London, we just went to the sightseeing tour around London naik hop-on hop-off. Visited Big Ben for the first time, naik river cruise, tgk Tower Bridge of London, visited Buckingham Palace and some other places.Last stop p Trafalgar Square and skali ade Open Air Orchestra free admission.. best sgt tgk dorg perform. Mlm tu, Qasim my cousin and his wife, Nawal bwk kteorg dinner at Burger and Lobster near Soho London. actually, den yg request! haha.. .abah terpikat dengan kesedapan and freshness lobster dorg. sedap and recommended!


 

The next day, we took train from Kingscross to Edinburgh Waverley. We really enjoyed the scenic view along the way. lawa sgt. ni bukti. hehe..Kat scotland and London, we stayed in Travelodge. sgt selesa and at reasonable price. We then walked around Edinburgh city and took the hop-on hop-off jgk. bus bwk jln2 smpi Edinburgh castle and all the interesting places in Edinburgh(xingt da nama tmpt dia). For lunch, kteorg mkn buffet makanan Pakistan yg sedap. abah puji beriyani and kari dia.


Alhamdulillah syukur for everything.. Jazakumullahu khairan jaza' (May Allah reward you with goodness in abundantly) to everyone especially mama abah, abang2, saudara-mara, housemates, friends, seniors, juniors, teachers from SKTRJ, SMK KGV, KYUEM, UCC, all the doctors who taught us and all the patients who contribute a lot to this humble journey.Thank you so muchh..=smoga kejayaan di dunia ni lebih membawa diri dan ummah dekat kpd Allah..Aminn.. =)
             
Blk je cork, rehat, went to graduation dinner and graduation ceremony the nest day tu. All ready, capturing moments of happiness with family and friends. Settlekn kotak shipping and amek translated degree kt Brookfield. terkenang2 jgk memori dari first year jln kaki p Brookfield.


Going to Ring of Kerry for the fifth time makes me falling in love with Ireland even more. Then, we went to Cliff of Moher with Paddywagon tour. It was a day trip for each place and very convenient kalau mls nk drive around. lpastu, jln area Cork city je, beli some souvenirs and we flew back home.



Currently, kt umh baru je lps interview SPA last wednesday. Quite relieved it's over, hopefully dpt posting awl. xtau nk wt ape da kt umh..hehe..nk jd cikgu tuisyen xapply2 lg. ahaha..cuma, ade la tlg2 mama abh jg ank buah..part time babysitter and full time daughter. hee.. Some simple tips for those yg akn interview:

1. Prepare resume and other docs as early as possible.
2. READ current issues, revise your medical knowledge. Very important to cover Zika, denggi in and out and cover the main topics in your interest.
3. Practise speaking. (kalau spe xbiasa cm I ni)
4. Doa and tawakal. Dress well and be confident.
5. Research and keep on searching. I just http://jonayyf.blogspot.my/2016/01/how-to-apply-housemanship-in-malaysia_26.html as a guide start from applying sijil sementara MMC, KKM until interview SPA. really detail and very informative

Sharing my questions during the interview [LTD 6 Seremban]:
1. Why ireland?
2. In what way do you think msia should follow ireland in terms of the healthcare system? Diff between msia n ireland healthcare system?
-I said psl pharmacist dispense meds etc in Ireland. So, dato' tu keep on asking in msia who should dispense the meds, pharmacist or doctors? Why? Dia mcm argue yg pharmacist pn know their job and bg scenario:

"Patient came with cough and she asked the patient is it wet or dry cough? (meaning ade kahak ke x?) konon mcm provoke la pharmacist know their job too."

I answered, "That's true, but only doctors know about the disease. for example the cough could be due to TB, asthma and other serious condition which only doctors can rule out safely. Hence, doctors should dispense the medications in Malaysia. Bla bla bla"

3. Posting yg plg suke/ minat utk further specialize? Why?
4. Case scenario sbb pilih O&G:
As a first poster HO, patient dtg with 10 weeks pregnant comes with vaginal bleeding. Wat you gonna do?hx, differentials, mx?
5. Kalau 3rd trimester comes with vaginal bleeding, ape ddx and management?
6. Stress reliever?
7. Why there are many male O&G now?
Kdg2 dato tu provoke ckt2, so kne stand firm la.. Anyhow, in shaa Allah baik2 je..All the best sume

ADVICE FOR FINAL YEAR MEDICAL STUDENT UCC

Assalamualaikum, 


It’s been 2 months since I left Ireland for good and it’s August now. Before I forgot, let me just give some tips and tricks for final years UCC. Although, I’m not the best person to give advice, but just take it as a sharing experience for those who like to benefit.

Study hard, pray harder. First and foremost, congrats for making until this far. Just a few more months to go, keep striving hard to improve yourself. So, this post is dedicated for ye guys, hopefully it will benefit ye. Plan early and try to stick to it or at least improvise the plan.

Before going to each module, let me give a brief overview how final year gonna be like. Quoting from one of the senior, “In final year, you are learning the same thing from fourth year but a lot more stressful”. It’s because everyone is having high expectations on you including yourself. You have to start thinking like an intern now. Learn how to prioritise the differentials, management plans etc. Ask and you will get the answer. Don’t be afraid making of mistakes, because you will learn better from that. Be afraid of being complacent! Always challenge yourself.

During the rotations, be friendly and try make yourself useful to the team. Aim to present cases to consultants, registrar or interns every week so that you will get a hang of how to present cases concisely and from that you will know how to tackle the history and examinations better next time. Yes, you need to polish your examination skills thoroughly! Open up your heart and mind to critics, I know it’s hard. But, just take it positively for your own personal growth.

1.       Medicine:

It’s a huge module with the biggest credit, meaning you have to pay extra attention to it without neglecting others. There are General Practice, Geriatrics and Clinical Practice. Follow ward rounds and get involve in the team. I used OHCM, Pockets of General Practice(Prof. Bradley) and ABC Geri. Being a textbook person, I try to complement the big topics by reading Kumar and Clarks/Davidson and make some notes because I learn better when I make my own notes. But, it depends on your style of learning. (tp, kne speed reading ckt sbb panjang kn). Assesment is mostly from your consultants so, be proactive and impress them.

Find a partner to correct you and vice versa. Better if you make friends with the interns, and ask any patient with good findings. Mine, I got Mallow University Hospital for short case. Dpt A.fib, aortic stenosis, consolidation and MS. Doa byk2 consultant lembut hati and dpt case yg bole jwb.

2.       Research:

Try settlekn data collection and analysis as soon as possible especially gang yg away tu. So that, bole focus on the hosp rotations. Ble da siap sume, aim utk siapkan slides for presentation and REHEARSE EVERYDAY at least for those like me yg tak suke public speaking ni. Mintk advice kwn2 utk komen stle presentation and hntr kt supervisor utk cek slides for improvement. Then, you can focus on the write up which has 70% weightage. So, wt leklok ye? Lit review kalau sama dgn tajuk FYP, copy paste je xpe.. jgn wt last minit, nt cuak plk.

3.       Obstetrics and gynaecology:

The content is exactly the same dgn 4th year except that you will have to focus more on the favourite topics and read the guidelines. Read from RCPIàRCOGàNICE. The MCQs after rotation quite specific because they expect us to know the details. So, bce guidelines. Kalau xsume page pn, bce summary da ckp n hafal. Siapkn logbook n present case kt consultants every week. In the finals, jwb essay and ade long case. Again, practise makes perfect. I like to spot question based on the trends from previous years. Tp, risky ckt. So, ikt instinct msg2 la.. I used Impey for 0&G, pocket O&G and guidelines.

4.       Paediatrics:

Challenging because the content is very wide, just like adult medicine but in kids. So, plan carefully or else nt xsmpt bce ble msk finals. I used Sunflower (Illustrated Paediatrics) and pockets paeds mse ward rounds. Very handy to refer. For osce, practise and practise with your friends. Make sure to cover the history and master examinations thoroughly. Look at previous years and practise them. Make sure to master all the common conditions but not neglecting the serious ones like child abuse, neglect etc. During the finals, practise all the past years or at least read the feedback that Prof gave because it will gives you an overview of how Prof want the students to answer the questions.  One page of answer per question, so no room to ‘goreng2’if you know what I mean. Memorise the differentials for common topics and the investigation and management plan. All and all, they just want to know how safe are you handling kids as an intern.

5.       Surgery
Surgery in finals focused more on abdominal and vascular as well as other topics. So, read all of them because the MCQs are very specific. Follow all the ward rounds and ask questions. I used Alisdairscott notes, OHCM, and other books too(xigt plk). It’s better if you practise answering data interpretation questions too. The most important topics that there want you to master is how to resuscitate patients, especially scenario like post-op patients etc. In the finals, make sure practise a lot of potential short cases. For me, I got CUH, and the cases are paraumbilical hernia, mastectomy male patient, ascites and incisional hernia. Brush up your examination skills. For long case, kne make sure cover the history in sequence, mention all relavant positive and negatives. Mcm patient presented with abdominal pain, list of the differentials git, urology-uti, gynae etc. Last but not least, know the first line investigation.

Prof tony obrien suka tanya "if u can ask one question to this pt, what would u ask"..so kena terus ckp yg relevant cth for case of ascites due to liver disease, most common cause for Irish population is heavy alcohol intake. Spend time wisely kt hosp. bopek kata 2 minggu b4 finals, dia g hosp cari positive findings from patients..eg: renal transplant post-op pt. dtg kt pt, introduce sume, n tros inspect scar n palpate..krenye wt focused examination. so, dia advise start awl. study group from awl. final year mmg kne wt study group..2x sebulan.. doa3x..doa dpt examiner yg baik, dpt jwb dgn baik etc
